Clinicians were spending more of each appointment facing a screen than the person in front of them. The software, not the medicine, was the bottleneck.

Drafting notes from the consultation itself gave that time back. The clinician reviews and signs; nothing reaches the record unreviewed.

Designing for review, not trust

The interface assumes the draft is wrong until confirmed. Uncertain passages are flagged rather than smoothed over, because a confident wrong note is far more dangerous than an obviously incomplete one.

Outcome

Documentation time per appointment fell by roughly half. Clinicians reported the more important change: they were looking at patients again.
